Breast Augmentation: Q&A With Board Certified Plastic Surgeon Dr. Walter Bernacki

Q: As a plastic surgeon, what do you consider to be your specialty?

Dr. B: Plastic surgery is that field where we take fundamental principles from all the surgical specialties: neurosurgery, urology, trauma, etc., and then we formulate plans which are oftentimes very complicated. It is kind the ultimate form of specialization. Still, people often ask, “what do you do the most of, or what do you think you’re the best at?”
Reflecting on that, I think my area of expertise is really breast surgery. I probably do more breast reductions than anybody in the area, and I have an expertise in breast augmentation.

Q: What is your philosophy when it comes to breast augmentation?

Dr B: When patients come to me, we first listen to what it is that that patient wants. I’m not going to recommend a surgery if they’re here for something totally different. So, we focus on what it is that concerns them and then we formulate a plan. Number one, that plan takes into account the patient’s safety. Then we determine what is the simplest procedure that will achieve the results the patient wants, and oftentimes that is the least expensive or most economical option.
Another philosophy of my practice is that we want patients to have an improved aesthetic result, and we want to do it in a tasteful and anatomically balanced approach. We want their breasts to be in proportion with their waist and their hips, and we want to achieve a very natural look.

Q: What can potential patients expect from a consultation?

Dr: B: My goal is to put our patients at ease. When patients have a consultation with me, we are going to work together formulate a very specific plan to meet their needs.
When a new patient comes in, I talk to them about what they want and learn more about their daily activities. Are they athletic? Do they like to run or lift weights? What is their goal for the surgery?
After we have a chance to get to know them a little better, with the assistance of my female nurse, we examine the patient, and we take very specific measurements and anatomic photos so that we can formulate an operative plan.

Q: What would you say to someone who is interested in a breast augmentation, but might be nervous about undergoing the procedure?

Dr: B: Anytime we have a patient in either a consultation or when I meet with them the day of the surgery in the preoperative area, I let them know that for them to be fearful is natural. I would be concerned if they weren’t scared of going under general anesthesia and having a major procedure. So, I tell patients that although it’s normal to be scared, I have a high level of experience. I’ve done tens of thousands of these procedures. Even though I’ve done a lot of these procedures, I take each patient as an individual and understand that we want to get the best result and take the appropriate amount of time to do everything that we need to do to make things as perfect as possible.

Q: Are there limits to the kind of breast augmentation surgeries you perform? For example, can a patient expect to go from an A cup to a DD?

Dr B: It’s important for me to give all patients a realistic expectation of what is and is not possible. If someone comes in with all these qualifiers and descriptors that I think are maybe outside of realistic expectations, we start from there—what’s realistic and what might be a perfect result in my mind. If we don’t allow them to know what the end point is or what can be done, they might think the result is not as good as what they had in mind.
That being said, I will always work with the patient to create a plan that takes into account their unique needs and goals to give the best possible result.

Q: What should someone who is interested in a breast augmentation do next?

Dr: B: The best thing they can do is come in for a consultation and sit down to talk about it. Let me give them an evaluation and come up with a best plan for them.
